Insurance & Payment Information

At Ballard Dentistry, we strive to make high-quality dental care accessible and easy to navigate. We work with many major insurance providers and are happy to help you understand and maximize your benefits.

Insurance Plans Accepted as Payment

We accept the following insurance plans and will bill them directly. Any remaining balance not covered by your plan will be your responsibility:

• MetLife
• Cigna
• Aetna
• Guardian
• UnitedHealthcare

Our team will submit claims on your behalf and help you understand your estimated coverage before treatment whenever possible.

Plans Requiring Upfront Payment & Direct Reimbursement

We also welcome patients with the following insurance plans. Due to the structure of these policies, payment is required at the time of service. Your insurance company will then send reimbursement directly to you:

• Delta Dental
• Blue Cross Blue Shield
• Anthem
• SIDS
• ASO
• UFT

As a courtesy, our office will provide all necessary documentation and submit claims to help you receive your reimbursement as quickly as possible.

What This Means for You

We will always review treatment costs with you in advance.
We accept Visa, Mastercard, American Express, CareCredit, HSA, FSA , Check, Cash, or Money Order as payments.
